The strongest evidence is the PREDIMED trial (Spain, published 2013 and 2018), which compared a Mediterranean diet supplemented with either extra virgin olive oil or nuts against a low-fat control diet, in over 7,000 participants at high cardiovascular risk. The olive-oil arm used roughly four tablespoons (50ml) a day — far more than a casual drizzle.

What actually defines the diet

Why the oil quality matters here specifically: the polyphenol content that shows up in cardiovascular research is concentrated in unrefined, unblended extra virgin oil — a refined "olive oil" or a blend loses most of it. What happened to the group that used nuts instead

PREDIMED actually ran two active intervention arms against the low-fat control: one supplemented with extra virgin olive oil, one with mixed nuts. Both showed reduced cardiovascular events relative to control, which is itself informative — it suggests the pattern's benefit comes partly from what it displaces (refined carbohydrates, low-quality fat) and not solely from one specific ingredient.
